Description Providence is calling a Community Outreach Coordinator - Medford (1.0 FTE, Day) to Providence Child Center in Portland, OR Apply today! The Community Outreach Coordinator works directly with Administration, Mission, Marketing/Communications, Foundation, local and county civic and social service groups/agencies/government on community outreach activities and programs aimed to assure the integration of Providence mission values and strategic goals through the coordination, provision and support of community based programs. The Community Outreach Coordinator is responsible for coordinating and supporting the operations and events of Swindells, its volunteers, vendors and other collaborators. In this position you will have the following responsibilities:

  • Timely response to requests for information from families, caregivers and service providers (related to disabilities, early childhood development and developmental delay).
  • Assist families, caregivers, and providers in locating specific resource materials or information within the Swindells Center library, the community, or online; and refer families to vetted and confirmed community resources that can meet the identified needs of their child.
  • Maintain knowledge of and connection with state and community-based agencies serving children and youth with special needs, including eligibility, current contacts, waiting lists, and financial requirements to provide up to date resources to families.
  • Provide social service and medical system support and coordination to families with a new diagnosis.
  • Serve as liaison with pediatric/family medicine clinics to promote use of Help Me Grow, including use of continuous quality improvement strategies and producing regular reports on referrals and outcomes to clinics.
  • Establish and foster collaborative working relationships with other community agencies, to include Early Learning Hubs, Parenting Education Hubs.
  • Adheres to the philosophy of the Mission and Core Values of Providence Health & Services. Required qualifications for this position includes:
    • Current and valid Oregon Driver's license
    • Two (2) years of Experience in community outreach coordination or promotion Preferred qualifications for this position include:
