Then, we want to hear from you!Adults and People with Disabilities (APD), part of the Oregon Department of Human Services, is looking for one dedicated and compassionate individual for a Human Services Case Manager position located in the Gold Beach office on the beautiful Oregon Coast.OUR AGENCY MISSION"To help Oregonians in their own communities achieve well-being and independence through opportunities that protect, empower, respect choice and preserve dignity."ABOUT THE PROGRAM Click here to learn more about Aging and People with Disabilities.WHAT YOU WILL BE DOINGThis position determines initial Title XIX social service eligibility, to develop and implement case management plans, to coordinate the provision of services for community-based care, and nursing homes, and to regularly review and update Title XIX financial eligibility and monitor social service plans. The local office team provides social, financial, and protective services to recipients of Title XIX.WORKING CONDITIONSThis position works in an office environment with daily use of personal computer/laptop and associated local and network environments. The position can sometimes deal with challenging customers, and a variety of physical and sensory situations, i.e. individuals who smoke and/or wear fragrances. Employee may be required to use a state vehicle to conduct business including home visits; must have a valid driver's license and acceptable driving record or be able to provide a suitable alternate method of transportation.
