30,000 patient visits each year at four health centers and via

provider of medically accurate sexuality education for young people
and adults, as well as training programs for professionals who work

breast and cervical cancer screenings, well-woman annual exams,
birth control, abortion care, LGBTQ health, STI prevention, testing

www.ppsworegon.org . Position Summary The Registered Nurse,
Centralized Follow-Up Team (RN CFT) position is dually responsible
for both follow-up duties and in-clinic skills. With a primary
focus on Centralized Follow-Up duties such as utilizing
telecommunications to coordinate patient follow-up care plans,
communicate lab results, and provide triage, the RN CFT will also
focus on in-clinic skills at least one day per week. The RN CFT is
regularly required to provide direct clinical reproductive health
care while adhering to Medical Standards and Guidelines, Clinical
Policies and Procedures, and RN Standing Orders. RESPONSIBILITIES:
Essential Duties Create and manage patient follow-up care plans;
Consult with Patient Services Leadership to confirm clinical
accuracy and appropriateness as needed. Maintain ongoing
communications with clinicians and other health center staff to
ensure continuity of patient care. Provide trauma-informed clinical
assessment, triage, and education via telephone consultations with
PPSO patients. Respond to patients who call with questions about
results, procedures, referrals, records, and others, as assigned.
Monitor and manage database of unassigned/unmatched labs to ensure
appropriate follow-up. Follow-up on internal and external patient
referrals with accurate documentation. Ensure response to requests
occurs in a timely manner. Review and sign off normal/Negative lab
results in the EHR; Notify patients of abnormal test results and
provide education of condition and treatment or follow-up
instructions. Educate patients regarding issues related to
sexuality, STl's, birth control, pregnancy-options, reproductive
health issues, vasectomies, and other interventions. Document
patient interactions, assessments, interventions, and outcomes in
electronic health records (EHR). Actively participates in the
delivery of reproductive health services, including but not limited
to abortion care, medication counseling and dispensing, consenting,
and screening. Limited ultrasonography performance and gestational
dating may be required. In-clinic training is provided. Education,
screening, and patient assessment for minimal and/or moderate
sedation. May be expected to administer moderate sedation, provide
recovery care and discharge. Peri-procedural and post-procedural
vitals monitoring and interventions. Review and sign charts to
confirm compliance with Planned Parenthood protocols and medical
standards. Provide STI treatments, lab orders, and refills per RN
Standing Orders. Handle, administer, and waste drugs in compliance
with PPSO Pharmaceutical policy and the DEA. Assists/performs IV
access/medication, injections, immunizations, CLIA waived
laboratory tests, RH typing, and phlebotomy. Other duties as
assigned. Equity and Culture Participate and engage in PPSO DEIB
initiatives. QUALIFICATIONS AND EXPERIENCE: The responsibilities

license and certification status. Minimum Required Qualifications
Requires a valid Oregon State Nursing license. Current American
Heart Association Basic Life Support (BLS) certification. Required
to obtain ACLS certification within 6 weeks of hire. At least one
(1) year experience in Community Health or Reproductive Health.
Preferred Qualifications Previous experience with Planned
Parenthood affiliates. Proficiency in Spanish preferred. Knowledge
of insurance types. Experience reading and interpreting safety
rules, operating and maintenance instructions, and policy/procedure
manuals. Familiarity with the importance of maintaining
confidentiality; HIPAA. Proficiency with computers including data
entry and Microsoft Office. Experience with appointment scheduling.

Travel & Availability Requirements Must reside within a reasonable
