Details - Home health setting serving Medford, OR and surrounding areas - 8-hour day shifts, 5 days per week (4x10 and 3x12 schedules are not available) - Visits up to 6 patients per day; 30 productivity points is standard - 1:1 patient ratio - Charting in Epic and OASIS - No weekend or on-call requirements - No floating between like units within the same assignment Requirements - Minimum 1 year of skilled home health PT experience required - OASIS experience required - Epic EMR experience required - BLS certification required - Personal reliable transportation required - Primary COVID-19 vaccine series plus one booster required - First-time travelers will not be considered for this position - Minimum of 2 supervisory references covering 1 year of employment within the last 3 years in a comparable specialty or setting required - SSN and DOB required for consideration - License required if applicable Additional Information - Responsibilities include evaluation and treatment of home health patients across Medford, OR and surrounding areas - Scheduling is organized to minimize drive time, though occasional travel of 50 miles may be required - Any color scrubs are acceptable - Local candidates within a 50–75 mile radius are welcome to apply at the same rate - Former permanent staff must wait at least 3 months before returning as a traveler and must not reside within 50 miles of the submitting facility - Travelers are expected to float between all facilities within the state; refusal to float may result in assignment termination - RTO requests should be disclosed upfront, as post-offer RTO requests may be denied - Parking is free American Traveler Job ID P-687961.
