Current Physical Therapy license in Oregon is required to start the assignment. Current BLS/CPR certification required. Must have a current driver's license, vehicle insurance, and access to a dependable vehicle for patient visits. Experience providing home health physical therapy preferred. Ability to complete comprehensive evaluations, develop and document PT plans of care, and submit progress notes per agency policy within required timeframes. Experience supervising PTAs and home health aides preferred, if allowed by state regulation. Schedule flexibility required, including participation in a weekend rotation and potential availability on observed holidays. Assignment Details Facility type: Home Health.
