Med/Surg LPN – Ready for Discharge Unit (RDU)/n - Location: Klamath

Setting: Acute Care Hospital – Med/Surg / Discharge Unit/n -
Contract Type: Travel Assignment (Travel LPN / LPN travel job)/n -
Start Date: March 1, 2026/n - End Date: May 31, 2026/n - Duration:
13 weeks/n - Guaranteed Hours: 36 hours per week/n - Shift: 12-hour
nights (every other weekend required) – night shift LPN/n -
Charting: EPIC electronic health record (EHR) – EPIC experience
preferred/n - Estimated Weekly Pay: $1,530 – $1,610/n /n/nJob
Requirements / Qualifications/n /n - Current Oregon State LPN
license (or eligibility to obtain) – LPN license in Oregon
required/n - Current BLS (Basic Life Support) certification/n - At
least 1 year of acute care nursing experience required (Med/Surg or
similar setting)/n - Proficiency in basic nursing skills, focused
patient assessment, and accurate documentation/n - Experience or
knowledge of discharge planning processes, patient education, and
care transitions/n - Familiarity with EPIC or other electronic
health records (EHR) is highly desirable/n - Strong communication,
teamwork, and interpersonal skills for interdisciplinary
collaboration/n - Flexibility and adaptability in a dynamic
healthcare environment focused on discharge readiness/n - Physical
ability to frequently stoop, bend, reach, stand, walk, sit, and
lift at least 50 pounds/n /n/nPrimary Responsibilities/n /n -
Provide direct patient care as a Licensed Practical Nurse under RN
supervision for patients medically ready for discharge/n - Perform
focused assessments and implement prioritized interventions as
directed by the RN/n - Develop and carry out focused plans of care
addressing patient needs related to discharge readiness and safe
transitions home/n - Monitor and evaluate patient responses to
interventions and progress toward discharge goals/n - Accurately
document assessments, interventions, and patient progress in EPIC
(EHR documentation)/n - Assign and supervise tasks to CNAs and
unlicensed assistive personnel as appropriate/n - Assist with
patient and family education, reinforcing discharge plans,
medication instructions, and follow-up care needs/n - Communicate
effectively with the interdisciplinary team (physicians, RNs, case
managers, social workers, PT/OT) to ensure continuity of care/n -
Adhere to hospital policies, procedures, and regulatory standards
to support safe patient discharges/n - Participate in unit-based
quality improvement initiatives to enhance patient outcomes and
