education, and overall qualifications. Position Summary The Mobile
Integrated Healthcare (MIH) EMT is a key member of a
multidisciplinary team providing proactive, post-hospital, and
community-based care in non-traditional settings. This role
supports continuity of care through in-home visits, patient
assessments, clinical and social screenings, patient education, and
coordination with healthcare providers and community partners. The
MIH EMT works both independently and collaboratively to improve
patient outcomes, reduce unnecessary emergency department
utilization, and increase access to appropriate levels of care.
Work Environment & Schedule - Non-traditional care environments
including patient homes and community settings - Shift work
includes seven 12-hour shifts in a two-week period Required
Qualifications Education - High school diploma or equivalent
required - Associate degree in health, human services, or related
field preferred Experience - Minimum of one (1) year experience as
an EMT on a 911 transporting ambulance or equivalent community
health experience - Behavioral or mental health experience
preferred Licenses & Certifications - Nationally Registered EMT (or
proof of equivalent continuing education) - Valid Oregon EMT
License - Valid Oregon Driver’s License - BLS and CPR
certifications - Community Health Worker (CHW) certification
required within 12 months of hire Key Responsibilities - Deliver
high-quality patient care in accordance with clinical guidelines,
protocols, and standards of care - Perform in-home assessments,
proactive and post-discharge follow-up visits, and specialty MIH
visits - Participate in the Mobile Crisis Response team for
behavioral health and crisis-related calls - Conduct point-of-care
testing (per protocol), medication reviews, wound care, and home
safety assessments - Provide patient and family education related
to chronic disease management (e.g., diabetes, CHF, COPD,
hypertension, fall prevention) - Assist with coordination of care,
including appointments, follow-ups, and alternative transport
arrangements - Observe, document, and report patient conditions,
treatment responses, and significant changes to providers - Provide
emergency medical care within EMT or CHW scope of practice and
assist with non-emergent referrals - Serve as BLS backup on
emergent and non-emergent 911 ambulance calls as requested -
Utilize telehealth and technology platforms for physician
consultation and longitudinal patient monitoring Please note that
