Job Description:
Hospice and Palliative Medicine Physician Are you passionate

Join our dedicated team as a Hospice and Palliative Medicine

patients. This role offers the opportunity to work in a dynamic
environment, combining both onsite and telehealth services, with a
focus on delivering comprehensive hospice and palliative care
tailored to individual needs. Key Responsibilities: Provide
compassionate hospice and palliative care to adult patients.
Conduct assessments, symptom management, and care planning.
Collaborate with interdisciplinary teams to ensure optimal patient
care. Manage calls and admit patients during scheduled on-call
shifts, including evenings, nights, and weekends. Participate in
telehealth consultations, IDT meetings, and complex case reviews.
Required Skills: Active Oregon medical license and DEA
certification. Board Certified or Board Eligible in Family Medicine
or Internal Medicine with substantial hospice experience. Proven
ability to manage complex symptoms and provide end-of-life care.
Excellent communication and interpersonal skills. Ability to work
independently and as part of a multidisciplinary team. Nice to Have
Skills: Prior experience in hospice or palliative care. Experience
working in telehealth environments. Ability to work flexible hours,
including weekends and call coverage. Preferred Education and
Experience: MD or DO degree from an accredited medical school.
Completion of an ACGME-accredited residency. Board certification in
Family Medicine or Internal Medicine. Minimum of 2 years hospice or
palliative care experience. Other Requirements: Willingness to work
onsite as needed, with potential future requirements. Availability
for scheduled on-call shifts, including 4-6 nights and
approximately one weekend per month. Commitment to high-quality
