Job Description Job Description Annual equivalent of $82,492 to
$102,876 (prorated by FTE). Oregon Licensed Clinician (LCSW, LMFT,
LPC) - this position includes a $3.00/hr license differential that
is equivalent to an additional $6,240 per year (prorated by FTE).

for Southern Oregon is a Certified Community Behavioral Health
Clinic (CCBHC) that specializes in providing services that are both
trauma-informed and recovery-oriented to persons/consumers with
mental illness and addictions. Trauma-informed care includes
safety, choice, collaboration, trustworthiness, and empowerment for
clients. The Care Coordinator is a healthcare professional who will
be responsible for facilitating communication and collaboration
with healthcare providers from emergency departments, hospitals,
and other acute or sub-acute, or residential placements to ensure
access to services, a seamless transition of care, and optimal
health outcomes for our clients. The Care Coordinator will have a
strong understanding of the healthcare system, including hospital
operations and patient care protocols. Responsibilities Coordinates
client care activities among multiple healthcare providers, both
externally and internally. Serves as a liaison between clients and
hospitals, ensuring that patients receive timely and appropriate
follow up services and connections to resources upon discharge.
Schedules discharge appointments and makes appropriate referrals
within agency resources. Tracks client progress upon discharge to
ensure engagement in services. Work to remove barriers to care. May
complete admission assessments to the crisis resolution center.
Works with treatment teams to modify service plans to address
current client situation. Monitors client progress during
hospitalizations by participating in treatment team meetings and
reading relevant documentation. Provides education and resources to
patients and families regarding available services and care
options. Facilitates discharge planning for clients transitioning
from the hospital to home or other care settings. Maintains
accurate and confidential patient records, documenting care
coordination activities and patient status. Collaborates with a
multidisciplinary team to evaluate and improve care coordination
processes. Qualifications Master’s degree in counseling, social
work, or a related behavioral health field from an accredited
college or university or Registered Nurse (BSN required). Must be a
QMHP on the Mental Health & Addictions Certification Board of
Oregon (MHACBO) registry. OR Meet eligibility qualifications to
register with MHACBO (please visit www.mhacbo.org for more
information) OR Must be registered as a licensure intern with the
State of Oregon or be licensed in Oregon as an LMFT, LCSW, LPC or
clinical Psychologist. Possess business experience sufficient to
meet the demands of the position, including proficiency with a
variety of software applications. Must pass state-required
background and DMV checks; possess a valid OR driver's license and
a satisfactory driving record. Options for Southern Oregon provides
