Job Description Job Description The Pavilion at Crescent Lake in Florida is seeking an experienced MDS Coordinator to join the leadership team. The MDS Coordinator plays a critical role in overseeing resident assessments, ensuring accuracy in documentation, and supporting optimal reimbursement and compliance. Key Responsibilities Complete and manage the Minimum Data Set (MDS) process in accordance with federal and state guidelines. Collaborate with nursing staff, therapy, and interdisciplinary teams to ensure accuracy of resident assessments. Monitor care plans to align with residents’ needs and regulatory requirements. Ensure compliance with CMS regulations, PDPM, and quality measures. Train and support staff in MDS documentation and related processes. Assist in maintaining survey readiness and participate in quality improvement initiatives. Qualifications Active RN or LPN license in Florida required. Prior experience as an MDS Coordinator in a Skilled Nursing Facility strongly preferred. Knowledge of PDPM, care planning, and long-term care regulations. Strong organizational, analytical, and documentation skills. Ability to work collaboratively with interdisciplinary teams. What We Offer Competitive compensation package based on experience.
