Job Title: Case Manager RN Profession: Registered Nurse Specialty: Case Management Duration: 13 Weeks Shift: Day Hours per Shift: 8 Experience: Minimum 1 year of Case Management experience required. License: Active nursing license required. Certifications: BLS required. Must-Have: - Acute hospital experience. - Experience with discharge planning and care coordination. - Familiarity with HIPAA guidelines. - Knowledge of medical necessity and continued stay reviews. - Ability to work in interdisciplinary teams. Description: This is a Case Manager RN position in a short-term acute care facility. The role involves supporting patients and families to create and fulfill discharge plans. Case Managers work closely with community partners to arrange post-acute placements. Utilization review experience is helpful, though not required. Weekend coverage will focus on discharges and new assessments only. The role requires floating to various acute departments within the facility as needed. The Case Manager RN assists with various tasks using support staff as needed. Patient age groups served include adults and geriatrics. Daily patient census ranges from 95 to 110. This position may cover inpatient units or emergency departments. Flexible scheduling is required to accommodate patient needs and staffing changes. Candidates must be adaptable and willing to collaborate with various teams. A vehicle is necessary for this assignment.
