Job Description Klamath Falls, OR Are you a passionate Physical Therapist seeking new opportunities in Klamath Falls, OR? within a primary care setting? We are seeking a compassionate Physical Therapist to work onsite in a primary care setting with Medicaid and Medicare population. Paid Time Off Schedule: 8am to 5pm, Monday to Friday What you will Do: Evaluate patients’ physical conditions and develop individualized treatment plans in collaboration with each patient. Appropriately code procedures and physical therapy diagnoses. Document all patient evaluations, treatments, and progress notes accurately in the medical record. Develop proficiency in E-ClinicalWorks EMR software, seeking assistance from staff or support as needed. Safely operate therapeutic equipment including exercise machines, modality devices, and manual therapy tools; seek guidance as needed to ensure proper use. Provide patient education regarding treatment plans, exercises, and ongoing care needs. Requirements: Current, valid license to practice Physical Therapy in the State of Oregon, or eligibility to obtain one. Current BLS certification. Working knowledge of legal, administrative, and clinical practices relevant to FQHC settings.
