Hospice with Providence Home Health in Medford, Oregon Position
Details: - Facility: Providence Home Health – Medford, OR - Start
Date: July 14, 2025 - Setting: Home Health & Hospice (field RN) -
Shift: 5x8 Days (08:00 AM – 4:30 PM) - Weekly Schedule: Monday to
Friday – No weekends, no on-call - Visits: Up to 6/day | ~30
productivity points - Charting System: EPIC OASIS - EMR Experience:
OASIS required - Scrubs: Any color ? Requirements: - Active RN
license (OR) – pending license accepted - BLS certification - 1
year of skilled home health hospice experience - Own transportation
required - COVID vaccine booster required - Not open to first-time
