Description Licensed Practical Nurse (LPN) - Home Health
Services at Providence Medford Medical Center Works under the
direction of a registered nurse to provide quality nursing care to
patients in their homes. Assists with collection of assessment data
and provides input to the planning of care. Implements plan of care
within the scope of licensure. Performs all duties in a manner
which promotes team concept and reflects the Providence mission and
philosophy. Required qualifications:
- Graduate of an accredited Licensed Practical Nurse
(LPN)/Licensed Vocational Nurse (LVN) school of nursing
- Current Oregon Licensed Practical Nurse (LPN) license
- Cardiopulmonary resuscitation (CPR) certification
- Current AHA-BLS (Basic Life Support for Health Care Provider or
ACLS) within thirty (30) days of hire
- Valid drivers license and reliable transportation
- Driving record consistent with GOP Safe Driving Policy 330
Incident Point Matrix at time of hire
- One (1) year of experience in nursing; long term care, acute
care, ambulatory care or home health and hospice care Preferred
