Job Description:
Job Description This full-time position will join our Medford
team and provide quality services in a team-based patient-centered
medical home setting with special emphasis on care for persons who
experience barriers to accessing care due to income, language,
cultural, and social barriers. Within the nurse practitioner or
physician assistant scope of practice, provide medical care and
education focused on individual and family engagement and
behavioral change, treatment compliance, and health risk factor
reduction. Utilize professional skills in diagnosing, treating and
managing acute and chronic health problems. EDUCATION: Master's
Degree in Nursing or a Doctorate in Nursing from a CCNE (Commission
on Collegiate Nursing Education) or NLNAC (National League for
Nursing Accreditation Commission) accredited graduate nursing
program OR Graduate of an accredited Physician Assistant Program
EXPERIENCE: Minimum two years' as a Nurse Practitioner or Physician
Assistant serving uninsured / underinsured and minority patients
preferred. Bilingual English/Spanish language skills desirable.
Must be able to document in an electronic medical records program,
EPIC preferred. LICENSURE AND CERTIFICATIONS: --- Current license
by the Oregon Board of Nursing as a Nurse Practitioner with
prescribing authority OR current unencumbered Oregon license to
practice as a Physician Assistant with prescription authority ---
DEA and National Provider Identifier (NPI) number --- Current Basic
Life Support Certification --- National Certification for FNP from
the American Association of Nurse Practitioner Certification
Program --- Eligible for PrimeCare credentialing Rogue Community
