Job We are looking to hire an Outpatient Case Manager for our
DownTown Clinic in Medford. Outpatient Case Managers provide
outpatient recovery support services to clients with severe and
persistent mental illness in a fairly independent work environment.
Case Managers work with a multi-disciplinary team in identifying
client needs and determining appropriate community resources, as
well as assisting with coordination of transportation and
healthcare arrangements. The Outpatient Case Manager provides
skills training to promote successful community living. Successful
candidates will be a team player and have the capability to
organize work, set priorities, meet critical deadlines, and follow
up assignments with a minimum of direction. Outpatient services
staff will receive crisis de-escalation training, which requires
performing various physical, hands-on maneuvers and techniques.
Travel within the community will be required. Work Schedule: Monday
through Friday, 8:00am - 5:00pm (Full Time) What You'll Make $24.00
- $26.00 per hour DOE/Credentials. Additional 5% Language
Differential offered for Bilingual or Multilingual candidates
(Spanish/English desired). About the Program Our Outpatient
Services program provides assessment and evaluation; treatment plan
development; and clinically indicated outpatient therapy, case
management, skills training, and psychiatric (medication) services.
ColumbiaCare Services is a qualifying employer for the Public
Service Loan Forgiveness (PSLF) Program. QMHA employees are also
eligible to apply for the Oregon Behavioral Health Loan Repayment
Program. What You'll Need You must be registered as a QMHA through
MHACBO or become registered within 30 days of hire. Eligibility for
QMHA registration requires 1 out of 3 Criteria below:
- Bachelor's degree in Behavioral Science: Psychology,
Counseling, Social Work, Human Development, or related field;
- OR minimum three (3) years of verifiable behavioral health
experience working with individuals with behavioral health needs in
a supervised setting;
- OR equivalent combination of behavioral science education in
addition to satisfactory supplemental behavioral health experience
and training Successful applicants MUST also have a valid Oregon
driver's license, maintain a safe driving record, and the ability
to pass a DHS criminal background check. About Us ColumbiaCare
