hire an Outpatient Case Manager for our DownTown Clinic in Medford.
Outpatient Case Managers provide outpatient recovery support
services to clients with severe and persistent mental illness in a
fairly independent work environment. Case Managers work with a
multi-disciplinary team in identifying client needs and determining
appropriate community resources, as well as assisting with
coordination of transportation and healthcare arrangements. The
Outpatient Case Manager provides skills training to promote
successful community living. Successful candidates will be a team
player and have the capability to organize work, set priorities,
meet critical deadlines, and follow up assignments with a minimum
of direction. Outpatient services staff will receive crisis
de-escalation training, which requires performing various physical,
hands-on maneuvers and techniques. Travel within the community will
be required. Work Schedule: Monday through Friday, 8:00am - 5:00pm
(Full Time) What You'll Make$24.00 - $26.00 per hour
DOE/Credentials. Additional 5% Language Differential offered for
Bilingual or Multilingual candidates (Spanish/English desired).
About the Program Our Outpatient Services program provides
assessment and evaluation; treatment plan development; and
clinically indicated outpatient therapy, case management, skills
training, and psychiatric (medication) services.ColumbiaCare
Services is a qualifying employer for the Public Service Loan
Forgiveness (PSLF) Program. QMHA employees are also eligible to
apply for the Oregon Behavioral Health Loan Repayment Program. What
You'll NeedYou must be registered as a QMHA through MHACBO or
become registered within 30 days of hire. Eligibility for QMHA
registration requires 1 out of 3 Criteria below: Bachelor's degree
in Behavioral Science: Psychology, Counseling, Social Work, Human
Development, or related field; OR minimum three (3) years of
verifiable behavioral health experience working with individuals
with behavioral health needs in a supervised setting; OR equivalent
combination of behavioral science education in addition to
satisfactory supplemental behavioral health experience and training
Successful applicants MUST also have a valid Oregon driver's
license, maintain a safe driving record, and the ability to pass a
