About the Job We have an exciting new opening for an Outpatient Certified Recovery Mentor (CRM) Case Manager to work out of our DownTown Clinic in Medford, Oregon. The Outpatient CRM Case Manager will provide outpatient recovery support services to clients with Substance Use Disorders or Co-Occurring Disorders who reside in Jackson County, works with a multi-disciplinary team in identifying client needs and determining appropriate community resources, and assist with coordination of transportation and healthcare arrangements. The Outpatient CRM Case Manager also provides skills training to promote successful community living; links individuals to self-help support groups; and connects them with resources to support their recovery including housing, employment, schooling, and professional and non-professional services. The position will assist clients in navigating different and sometimes difficult systems and services (e.g., child welfare, criminal justice, mental health, primary health, probation and parole, and the court systems) and in obtaining necessary documents such as Food Handlers Cards and ID's. They will also facilitate a recovery skills group and/or co-facilitate evidenced-based recovery curriculum. The Outpatient CRM Case Manager will transport clients to or meet clients off-site for community support meetings such as AA, NA, or other 12-step functions to support clients in making connections. Work will generally occur in an outpatient services clinic setting, residential treatment program, and supportive housing programs. Work Schedule: Full Time (40 hours per week), schedule TBD What You'll Make$24.00 - $26.00 per hour DOE/Credentials. Additional 5% Language Differential offered for Bilingual or Multilingual candidates (Spanish/English desired). About the ProgramOur Outpatient Program provides assessment and evaluation, treatment plan development, and provides clinically indicated outpatient therapy, case management, skills training, and psychiatric (medication) services. The Psychiatric Services Team operates as a branch within the Outpatient Program, specializing in psychiatric, nursing, and medication services. Psychiatric services are provided both on-site and remotely through telemedicine. ColumbiaCare Services is a qualifying employer for the Public Service Loan Forgiveness (PSLF) Program. What You'll NeedSuccessful applicants MUST:Be CRM I (or CRM II) certified through MHACBOHave SIX (6) months' experience as a case manager and providing CRM services Candidates must understand HIPPA and 42CFR Part 2, demonstrate strong communication and active listening skills, and have experience in the Harm Reduction model. Working knowledge of Microsoft Office and Electronic Health Record experience is a plus. Successful applicants MUST also have a valid Oregon driver's license, maintain a safe driving record, and have the ability to pass a DHS criminal background check.
